Patient Request Instructions for Medical Records
Welcome! Please Read Before Submitting Your Request
This online system allows patients to securely and efficiently
request their Medical Records from
Shasta Community Health Center.
Before submitting your request, please review the available
delivery options and required documentation to ensure a smooth
process.
Request Options
Option 1: Electronic Delivery (Preferred Method) – Fast & Secure

Your medical records will be delivered securely online.

Processing Time:
Up to 3 business days

Notification:
You will receive a text or email
when your records are ready for access.

Delivery Method:
Secure online access (No fax, email, or mail).

Delivery Format:
You will receive a PDF copy of your records.
Option 2: In-Person Pick-Up (Paper Format)

Pick up your printed records at:

Processing Time: 5-7 business days.

Notification:
You will receive a text or email when your
records are ready for pick-up.

Pick-Up Instructions:

Please allow 15-20 minutes upon arrival,
as records are printed at the time of pick-up.

A valid photo ID is required for pick-up.
Option 3: Mail Delivery (USPS Flat Rate Envelope)

Your medical records will be mailed via
USPS Flat Rate Envelope to the address provided
on your request - make sure it is current and accurate.

Processing Time: 15 business days
plus standard mail delivery time.

Notification:
You will receive a text or email when your
records are placed in the mail to you.

for lost, delayed, or misdelivered records by USPS.

By choosing this option, the patient
accepts all risks associated with mail
delivery.
Who Can Request Records?
Only the following individuals may request patient records:

Adult Patients (18+ years old)
-
Parents/Guardians of Minors
(must provide legal documentation)

Legally Authorized Representatives
(Healthcare POA, Conservators, Legal Guardians, Executors –
proper documentation required)

A parent may NOT request records for an adult child
(even if they are the insurance holder).
What You Need to Submit a Request
To ensure your request is processed quickly,
have the following ready before you begin:

Valid Photo ID
(Driver’s License or Government-Issued ID)

Legal Documentation
(if requesting on behalf of a minor or another patient)
